New Faction Tracker Sheet

I'm currently playing Deck of Fate with my home group through the Dolmenwood setting. One of my favorite strengths of the setting is the robust factions and their interactivity. While prepping, I stumbled across Among Cats and Books blog series on Dolmenwood factions and wanted a form that I could use at the table to keep track of faction turns all on one sheet.
